The Grievant Information Sheet should be used to give the UTLA Representative a detailed description of the grievance to be filed, dates and other pertinent information, remedy sought, etc. and attach a copy of any documentation (i.e. Inadequate Service Report, Below Standard Stull, Public Reprimand, etc). They can fax the form attention to their assigned Area Rep @ (213) 368-6256.
